Application Process

March 21, 2018


WHERE: At the school, your child would attend during the 2018-2019 school year if accepted                         


WHEN: March 21, 2018, between the hours of

7:30 a.m. – 2:30 p.m.


A child must reach the age of 4 years by August 15, 2018, to enroll in Preschool for the 2018-2019 school year.


      Items to bring:

 Certified Birth Certificate

 Social Security Card

 Immunization Record & Current Physician’s Physical Report (both are on the TN Dept. of Health Certificate of Immunization form)

 Proof of Residence (utility bill, rent receipt, etc.)

 Proof of Income

            DOCUMENTATION for ALL of the following that apply:


  • Siblings met eligibility for Free or Reduced Price Meal Program in the 2017-2018 school year
  • Food Stamps/EBT            
  • Families First (TANF)                     
  • Foster Care
  • Head Start
  • Homeless or Migrant                       
  • Unemployment                
  • Workman’s Comp                
  • Pension(s)                                         
  • Retirement
  • Social Security benefits     
  • Veteran’s benefits                             
  • Child Support
  • Alimony
  • SSI Disability
  • AFDC/Public Assistance Payment               
  • 2017 W-2’s or tax return or last 3 pay stubs from job(s) showing GROSS income
  • Any other income not included in the above
